Short term care

We can help you and your carer with short-term care to help you recover, regain your living skills and live independently at home after an illness or leaving hospital.

Short-term support from social care services 

To help you stay as independent as possible, we can help you with short-term support. 

We also offer a rehabilitation service after you leave hospital; we will talk to you about what help might be available and how you will manage at home. This support is provided for up to six weeks to help you regain any lost skills and confidence.

Get online advice to help you with everyday activities: self-assessment 

Use our free AskSARA online self-assessment tool to find out how you can help yourself with daily activities in your own home. You can do this instead of contacting us directly for a social care needs assessment.

Once you have completed the assessment, you will be provided with an online report showing how you can make adjustments in your home or the type of equipment needed to support you.

Support for people who care for you 

If you receive care from a family member, friend or neighbour, they may be eligible for support to help them care for you. To check their eligibility to receive support, they need to complete a carer assessment online.
